Position Information:
The Behavioral Health Discharge Specialist will assist the behavioral health team by coordinating discharge/transition planning services and providing clerical support by processing and disseminating the information and by performing departmental admin/clerical duties related to the execution of the discharge plan. The Discharge Specialist assists the Case Manager BH Licensed Clinician others on the interdisciplinary team and external post discharge providers to obtain services for the patients in preparation for discharge. Collaborates with the Network for Advanced Behavioral Health for successful connection and patient engagement regarding outpatient plans. Will arrange transportation and SDOH migration. The Discharge Specialist is skilled in establishing rapport with health care providers and community contacts. Anticipates and proactively avoids potential delays in disposition.
Experience Requirements:
1 Year Related Position Experience Required - And/Or 1 Year Behavioral Health Preferred
Education Required:
High School Diploma or Equivalent Required - And/Or Bachelor's Degree Social Work Preferred
