GENERAL SUMMARY
As a Medical Social Worker in the Social Services Department, you will play a pivotal role in providing support and assistance to individuals 18 years of age and older facing challenges accessing and identifying resources. You will work closely with interdisciplinary teams, patients, community agencies, and other stakeholders to develop and implement plans aimed at achieving optimal patient outcomes. Medical Social Worker role will encompass a wide range of responsibilities aimed at addressing the social and emotional factors that impact patients’ well-being and treatment outcome. The Medical Social Worker will provide assessment, advocacy, crisis and mental health support in a clinical and non-clinical setting. The Medical Social Worker will possess problem-solving skills in assisting patients navigating the health care system. Medical Social Worker will provide departmental support in other areas within the social services department such as Prescription Assistance, Resource Center and Social Work and various other duties as outlined to support the well-being of our patients within Cooper Green Mercy Health Authority. Medical Social Worker may also participate in community events outside of traditional work schedule.

KEY DUTIES & RESPONSIBILITIES
• Engage with communities to identify social determinants of health impacting individuals’ access to care and treatment outcomes.
• Provide additional support within the various social services departments such as Prescription Assistance, Resource Center and Social Work.
• Provide education and scheduling of Cooper Green transportation services
• Collaborate with community organizations, advocacy groups, and local resources to develop initiatives and programs aimed at addressing social issues affecting health outcomes.
• Conduct comprehensive assessments to identify psychosocial needs, case management and provide interventions tailored to the unique circumstances of each patient and family.
• Coordinating and Facilitating services to meet the complex needs of individuals and families.
• Advocate for patients’ rights and assist them in navigating the healthcare system, including facilitating communication with healthcare providers, and addressing barriers to care.
• Stay abreast of current trends, best practices, and evidence-based interventions in medical social work through ongoing training, professional development opportunities, and participation in relevant conferences and workshops.
• Maintain accurate and up-to-date documentation of patient assessments, patient encounters and service
coordination activities in accordance with department workflow and processes.
• Offer supportive interventions, conflict resolution and coping strategies to help patients and families navigate the challenges of their medical journey.
• Educate patients and caregivers about our prescription assistance program, resource center, resources, eligibility criteria, application processes, and resources for obtaining medications.
• Assist patients in accessing community resources, financial assistance, housing support, transportation services, and other resources to meet their needs.
• Ensure all documentation is accurate, up to date, and submitted in accordance with the Social Services Standard Operating Procedures (SOPs).
• Other Duties as assigned.

Position Requirements:
MINIMUM QUALIFICATIONS
This role requires a bachelor’s degree in social work or master’s degree in social work with 2-5 years of experience in social work, social services, case management or a similar role within a healthcare setting or related field. Must be licensed as a LMSW (Licensed Medical Social Worker). .

WORKING CONDITIONS/PHYSICAL DEMAND
Medical Social Worker will primarily reside in a general office area.

The role is for full time, with the following options as work hours:
7:00 am - 3:30 pm CST
7:30 am – 4:00 pm CST
8:30 am – 5:00 pm CST

This role may require lifting up to 50 lbs.
