Responsible for provision of pharmaceutical care to patients managed in the Infectious Diseases Program and Hepatology clinic. The specialist may also assist in other ambulatory practices as needed. Activities include evaluate, monitor and modify drug therapy, provide recommendations to optimize patient outcomes, minimize safety issues and insure cost effectiveness as well as provide Drug Information for medical personnel. The pharmacist will serve as a liaison with the outpatient pharmacy to ensure patients receive the prescribed medication. The pharmacist should be an effective communicator and be able to integrate themselves within the health team and current structure of the pharmacy department. Additionally, the candidate should coordinate pharmacotherapy activities and educational opportunities for department pharmacists and residents in the ambulatory practice setting. The clinical pharmacy specialist, if chosen, will act as a member of the Penn Specialty Pharmacy Quality Committee and devote approximately 10% of time to the committee. Finally, the clinical pharmacy specialist should fulfill requirements associated with hospital and department policies and procedures, JCAHO and other regulatory agencies.
Current pharmacy license to practice in the state of Pennsylvania.
Doctor of Pharmacy Degree in Pharmacy from an accredited College of Pharmacy.
Three to five years of experience in hospital pharmacy practice including precepting students and residents or similar experience in an academic setting preferred.
Completion of a Pharmacy Practice Residency and a Specialty Residency in Infectious Diseases required.
