Position Description:
Provides case management and therapeutics services with Clinician team member to Spanish speaking children and their families. In the home and community for children at high risk of hospitalization. Services include meeting with school personnel and other providers. The PHIICAPS team typically provides service to 8 children and their families averaging of 4-5 hours working with each family. Paid after-hours on-call responsibilities. Must speak Spanish.

Minimum Requirements:
Must posses a High school diploma or GED and a Bachelor’s degree with experience working with Spanish children and/or adolescents.
