The WOCN Assistant Team Leader is responsible for assisting the Penn Medicine at Home Director of Practice and Professional Development with the clinical operations of the WOCN home health team. In this role the WOCN Assistant Team Leader will function to support clinical operations and the provision of care delivery. He/She will review clinical documentation and participate in other quality and regulatory activites. The WOCN Assistant Team Leader will be responsible for conducting team care meetings, co-leading team meeting, and assisting the WOCN staff with day to day operations regarding patient care.
The WOCN Assistant Team Leader provides expert consultation in collaborative partnership with various stakeholders to design, implement, and evaluate wound/ostomy programs that uphold practice standards, support evidence-based practice, and improve the delivery of high quality, comprehensive, individualized patient and-family-centered care.
The WOCN Assistant Team Leaders provides leadership to identify the learning needs of the learning needs of the home health staff and responds to these educational needs while considering the effected patient populations, new clinical evidence, organizational goals, and changes both locally within the health system and nationally/globally. The WOCN Assistant Team Leader serves as a vital liaison to communicate and negotiate multiple perspectives within his/her organization in a respectful and effective manner to achieve/advocate positive change and delivery of appropriate and ethical care.
Required Education /Or Experience Experience:
Master's Degree Nursing and 3+ years Extensive clinical experience working with adults with wound/ostomy needs in various settings. Demonstrated leadership skills
Doctorate degree preferred.
Licenses, Registrations, and Certifications Required:
Currently licensed as a professional nurse in the state of Pennsylvania and/or New Jersey depending on geographic location and scope of responsibilities.
Possession of car, valid driver's license, and auto insurance
Certified Wound, Ostomy, and Continence
