Position Description:
The Home Care and Hospice After Hours Coordinator Quality utilizes clinical information and knowledge of patient care, operational processes, and information systems to effectively support the after-hours nursing team. The AHNC collaborates across teams to plan, implement, and evaluate effectiveness of clinical care to promote superior patient outcomes. The ANHC serves as the coordinator for the after-hours team of nurses providing home care, hospice care, and infusion care with attention to timeliness, quality of care, safety, and appropriate utilization of resources. The AHNC works closely with clinical operational leaders, schedulers, and others to assure admission visits and other urgent or scheduled visits occur. The AHNC participates in quality initiatives, The ANHC is an integral part of the Home Care and Hospice team and supports safe, evidence based care practices, utilization of resources, coordination of care, and documentation.

Minimum Requirements:
BSN required. PA RN license required.

5 plus years home health/ hospice/ infusion experience with leadership experience as a unit leader, charge nurse or manager.

Additional Information:
This individual will be working 1 PM- 9 PM Monday through Friday.
