In this role the Experienced CRNP will be working in Clinical Nutrition Support Services and will support patients in both the in-patient and ambulatory care setting. They will provide nutrition assessment and work with the clinical nutrition team and primary service to manage Parenteral Nutrition therapy and transition patients from the hospital to home. In total, 80% of the role will be to manage the home Total Parenteral Nutrition (TPN) patient population outside the hospital in conjunction with our consulting physician and Infusion Company/Penn Home Infusion Therapy. Patients are managed remotely from the Hospital of the University of Pennsylvania (HUP) offices and via a monthly clinic
This is a unique role which does not require experience in this field and the practice will train on the Nutrition/Home TPN aspects.
Graduate of a NLN- approved school of nursing.
RN and CRNP licensure in the State of Pennsylvania. Must have current CPR certification.
Completion of an approved Nurse Practitioner program with a MSN.
National certification is required.
Prior nursing experience in specialty is preferable.
Responsible for the nutritional care of UPHS patients. Functions as a CRNP and professional consultant to the physician to assure cost effective, high quality and current nutrition/nutrition support practices (parenteral and enteral infusion/access). Performs nutrition assessments and provides education on nutrition access/infusion. Develops specialized nutrition care plans and performs ongoing monitoring of infusion therapy. Coordinates the discharge planning of patients requiring home infusion therapy. Takes leadership role in development and implementation of departmental education programs (continuing education, education of students and professional visitors) and quality improvement/outcomes projects. Maintains comprehensive knowledge base in nutrition support (infusion/access) therapy. Evaluates, interprets and applies results of research to CNSS nutrition support practices and the delivery of patient care services. Participates in the development of research-based policies/procedure and practice guidelines. Is expected to present lectures or publications regarding specialized nutrition support within UPHS, local, state and national meetings
