Position Description:
Penn Medicine Princeton Health is seeking a per diem day shift Registered Nurse in the Acute Rehab unit.

Our Acute Rehabilitation Unit is a 17 bed unit located on the fourth floor of Penn Medicine Princeton Health, a leading teaching hospital and state-designated Primary Stroke Center. Our Acute Rehabilitation Unit is one of only 10 inpatient hospital programs in New Jersey to hold the CARF Accreditation distinction.

As a Registered Nurse in the Acute Rehabilitation Unit you will be responsible for assisting patients with medical and functional disabilities attain and restore maximum function through a patient-centered care approach. The nurse aids in the implementation of rehabilitation goals and treatment strategies that are concurrent with evidence-based practice.

We care for individuals with conditions resulting from:
• Amputations
• Complex joint replacements, including bilateral knee and hip replacements
• Craniotomies
• Debilitating rheumatoid arthritis
• Guillaine-Barré syndrome
• Hip fractures
• Major multiple trauma
• Medical complexity
• Multiple sclerosis
• Neurological conditions
• Parkinson's disease
• Stroke

Some job functions and duties will include, but are not limited to:

• Accountable for planning, delivering, coordinating, evaluating, and modifying the patient’s care for a defined period of time
• Uses evidence-based assessment techniques to collect and document comprehensive data pertinent to the patient’s health
• Analyzes the assessment data to determine diagnoses or issues.
rehabilitation nurse along with the interdisciplinary team follows the plan of care created by the rehabilitation physician, making revisions based upon the response to treatment of the persons served.
• Develops plan of care that is prescriptive of strategies and alternatives to attain expected outcomes in conjunction with persons-served/family and the healthcare team.
• Identifies expected outcomes for a plan individualized to the patients served, considering persons-served/family cultural values, ethics, environment, and other associated risks, benefits, costs and current scientific evidence.
• Coordinates implementation of the individualized plan of care and documents interventions and any modifications of the identified plan.
• Utilizes the plan to guide those involved in the care of the patient across the continuum
• Evaluates and documents progress toward attainment of outcomes, including the patient and others involved in the care or situation.
• In order to achieve the outcomes defined in the plan of care, the registered nurse will assure that persons served experience exceptional cooperation among all members of the health care team who treat them and each other with dignity, respect and sensitivity.
• Responsible for effectively transitioning plan of care with attention to continuity across shift and settings of care.

Minimum Requirements:
• Bachelor’s Degree in Nursing (BSN) from an accredited school of nursing or in pursuit of a BSN

Related Work Experience
• Greater than 15 months professional nursing experience
• Can additional experience substitute for education? If “yes”, specify type of experience (i.e. supervisory or nonsupervisory, etc) and identify how many years of additional experience:
• A registered nurse with an Associate’s Degree with greater than 5 years of clinical experience in a clinical specialty may be considered.

• Active New Jersey Registered Nurse License (required prior to first day of hire) required
• Basic Life Support Certification (required prior to first day of hire) required
• National Nursing Certification preferred
