Position Description:
The Clinical Resource Coordinator is responsible for performing integrated concurrent utilization management and coordinating a multidisciplinary plan of care to ensure the delivery of quality care that facilitates discharge and expected outcomes in a cost-effective and efficient manner. This will be accomplished by working with the Care Team, clinical pathways and other internal resources to meet the demands for information.  The Clinical Resource Coordinator collaborates with Physicians, Social Workers and other health care team members to develop a discharge plan of care and facilitate a timely discharge.

Minimum Requirements:
The qualified candidate must have a BSN and current Pennsylvania state RN license required.  Extensive clinical experience required.  Knowledge of utilization management and word processing preferred.
