Position Description:
Penn Medicine Home Health LPN’s provide exceptional care and truly make a difference in the lives of their patients.  Opportunities are available in the following specialties: Home Care, Home Palliative Care, and Home Hospice services.
We currently have open LPN positions across all 5 counties that we service. These include Bucks, Chester, Delaware, Montgomery and Philadelphia counties. Submit your application to discuss specific geographical locations with the recruitment team.

The Licensed Practical Nurse (LPN) functions collaboratively within the team by observing the patient/family needs and facilitating care. The LPN provides direct nursing care to home care patients within the scope of practice for LPNs. The individual will follow the Plan of Care (POC) established by the RN and the Interdisciplinary Team for patients.

Minimum Qualifications:
Graduate of an NLN accredited licensed practical nursing program. Pennsylvania and/or New Jersey LPN licensure depending on geographic location and scope of responsibilities.
2+ years of Full Time experience as an LPN in Acute care, home care, hospice care, or long-term care.
BLS/CPR Certification
Valid Driver’s License and Car
