Position Summary:
The RN Manager of Home Care is responsible for planning, designing, implementing, and maintaining programs, policies, and management systems to promote efficient and effective delivery of high quality care and to maintain financial viability of home care programs. Provides supervision and management of intermittent home care clinical programs providing 24/7 customer focused service delivery ensuring high quality, cost effective services that lead to best practice performance with maximum and timely reimbursement.
Qualifications:
EDUCATIONAL REQUIREMENTS
- Bachelor's Degree, Nursing (BSN) or
- Master's Degree in Occupational Therapy, Physical Therapy, Speech Language Pathologist
CERTIFICATION & LICENSURE REQUIREMENTS- Registered Professional Nurse (RN) License Illinois upon hire or
- Physical Therapist (PT) License Illinois or
- Speech Language Pathology (SLP) License Illinois or
- Occupational Therapist License Illinois (OT) and
- If RN - National Nursing Certification within 3 years of hire.
- If PT - National PT Certification within 3 years of hire.
- Driver's License upon hire and
- Proof of Auto Insurance upon hire and
- Basic Life Support (BLS) (AHA) within 30 days and
EXPERIENCE REQUIREMENTS- Three (3) years in Home Health and
- One (1) years in Leadership
ADDITIONAL REQUIREMENTS- Attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients.
- Continuing education requirements for this position will include content that is assigned by the organization to comply with DNV Hip and Knee Replacement Certification requirements. This content will only apply to those directly assigned to the total joint replacement practices. This requirement is subject to change pending updates to DNV Hip and Knee Replacement Certification standards.
SKILLS AND KNOWLEDGE- Knowledge of home health care and Medicare regulated home care services.
- Knowledge of the principles and practices of primary medical and nursing care.
- Knowledge and understanding of interpersonal and team dynamics.
- Ability to lead, motivate and develop a team in the provision of quality care and innovative programs.
- Knowledge of accreditation standards and demonstrated ability to interpret their application.
- Excellent verbal and writing skills.
- Public speaking skills and experience.
Essential Functions:
- Performs introductory, annual, performance appraisals for unit employees based on job description and performance standards.
- Ensures appropriate orientation, education, credentialing, and continuing professional development of personnel.
- Evaluates staffing needs and appropriate patient care assignment.
- Participates in recruitment, selection, and retention of personnel, including staff representation of the population diversity.
- Directs and manages personnel in assigned area.
- Is a role model in nursing, demonstrating competency in clinical judgment, select technical skills and knowledge base.
- Demonstrates clinical expertise in the execution of the nursing process
- Directs and assists staff in its application and evaluates level of care provided by the unit nursing staff.
- Plans and executes unit budget.
- Identifies and justifies variances on a monthly basis.
- Ensures patient rounds are completed on a daily basis.
- Monitors HCAHPS scores and develops and implements plans to correct scores as needed.
- Responds to action lines in a timely manner.
- Responsible for the continual monitoring and follow-up of scores related to the Carle Experience.
- Assumes responsibilities and obligations of Administrator in the event that the Administrator is unavailable.
