Position Information
Admission Representative is responsible for providing case management services to all patients referred to WakeMed Rehab. This includes conducting a thorough clinical evaluation, assessing available resources and facilitating the development of a discharge/transfer plan from the referral location to the appropriate program within WakeMed Rehab system. When admission to WakeMed Rehab is not appropriate the Admission Representative will provide recommendations for alternative care options. This position is also responsible for managing the planning, development and maintenance of an efficient admission system, establishing a community referral system and maintaining positive public relations with all internal and external contacts. Admission Representative functions as an advocate for the patient and works to identify initial goals for rehab and outcome achievement potential. This position is also responsible for determining how available resources can be used and making referrals to appropriate community/financial, support and advocacy resources. Admission Representative is responsible for verifying available healthcare benefits and is the primary point of contact for all insurance company case managers, providing communication as required for pre-certification of patients' hospitalization and treatment. The Admission Representative provides communication to all involved parties to facilitate continuity of care for each patient and ensures a smooth transition to the rehab program. The Admission Representative provides specific marketing activities to monitor and develop relationships with all referral sources. *Since the salary for this exempt position is pulled from a table assuming the position is full time, any position scheduled to work less than 80 hours a pay period will be prorated.
Experience Requirements
:
Two years experiences in acute or rehab medical field required. Marketing, sales or public relations experience preferred..
Education Requirements
:
Associate Degree in Nursing is required; Bachelors Degree is preferred..
Licensure/Certification Requirements
:
Licensure as a registered nurse in the State of North Carolina. Must have a personal vehicle and a North Carolina driver's license.
