Position Summary:
The Physical Therapist Team Coordinator administers to clients, according to physician’s orders, various evaluations and therapeutic procedures and maintains a client record in accordance with State and Federal Regulations.  This is a professional-level position operating independently.  Work situations are varied and require initiative, analysis, interpretation of information from a variety of sources, a service-oriented individual, and the use of discretion and good judgment in decision-making.

Minimum Requirements:
Minimum of one year of physical therapy experience required. Experience in Medicare Certified Home Health preferred.

Education/Licenses/Certifications:
Successful completion of a physical therapy program at a school of physical therapy approved by the American Physical Therapy Association or the Council on Medical Education and Hospitals of the American Medical Association or both. Licensed in the state as a physical therapist. Current CPR certification.
