Position Description
This Interdisciplinary Team position is responsible for providing pharmaceutical expertise in the areas of supportive care and symptom management for Hospice patients. The clinical pharmacist is also responsible for educational activities that support Hospice's strategic goals for professional community education and projects to support our mission.

Position Requirements
1. Successful completion of a doctor of pharmacy program or equivalent experience. 2. Specialty residency in oncology or related field preferred. 3. At least two (2) years discipline specific experience in Hospice, oncology, human services, or related field. 4. Currently licensed as a registered pharmacist in Florida. 5. Participation as a member of an Interdisciplinary Team preferred. 6. In depth current knowledge in area of specialty related to Hospice and/or palliative care. 7. Ability to translate organizational goals into effective programming. 8. Experience in clinical pathway development, including literature reviews, drug utilization, planning, implementation, and outcome evaluation preferred. 9. Commitment to research process and application to practice. 10. Strong interpersonal skills, communication skills, and public speaking experience. 11. Must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must be able to provide proof of automobile insurance, a copy of which will be placed in their employee file.
